How to Create a Multi-Page Form for Your WordPress Site?

As we have mentioned earlier, in this tutorial we will use the Live Forms plugin to create a multi-page form for the WordPress site. Now, you might be thinking that why we are using Live Forms for creating the multi-page form?

Because, where most of the form builder plugins provide the multi-page form feature in the premium version, Live Forms offer that feature for both free and paid version. Its drag and drop feature takes only a few seconds to build a form. Moreover, with the Live Forms, you just need to click a button to make your form a multi-page form, whereas other form builders make the process lengthy and tricky.

Step -1: Install and Activate the Live Forms Plugin

To start creating a multi-page form for your WordPress site, you need to install and activate the Live Forms. Live Forms provide the multi-page form feature on both free and paid version. So you can easily install the plugin from WordPress.org.

To install Live forms, navigate to “Plugins> Add New” and then search for the Live Forms plugin. Once you find the plugin, just install and activate it.

After activation, the “Forms” menu will appear on your admin dashboard.

Step- 2: Creating a Multi-Page Form WordPress Site

Now, it’s time to start creating the multi-page form.  For that, go to “Forms > Add New” and simply select a form template. If you don’t want to choose a template and start creating Formby yourself then click on “Start From the Ground”.

Once the form builder page appears, the first thing you need to do is add a title for the form. For this tutorial, we are going to create a multi-page School Admission Form.

As we have added the form title, you need to add form fields to your form. But before that, you need to go to the “Setting > Look and Feel” and check the “Enable multi-page form” option. It will allow you to create the multi-page form.

Now, to add form fields, check the “FIELDS” option on the left side of your screen. Live Forms offers three types of form fields to its users:

• Commonly Used Fields and Generic Fields are for both free and premium version users.

• Advanced Fields are for only premium version users.

For School Admission Form we need to use advanced fields also. If you need advanced fields then check the pricing page and purchase a plan.

Now, drag and drop the form fields you want to add to one group. For this School Admission Form, we have added a few form fields for the student information section.

After adding a section of form fields, you may want to split the form into pages. And creating a multi-page form into a single-page form is just a click away. Just drag the “Add New Section” button from the top right corner of the form builder page and drop it on the form.

You can also name the section or page. For that, hover over the section filed and the settings option will appear on the left side. Now, add the section name on the text box under the “Section Name”.

Next, add the additional form fields to complete your form and customize the submit button.

Lastly, click the “Save” button to save the form.

Step-3: Publish The Multi-Page Form

Once you are done, it’s time to add the form to a page or post. For that, click on the “Embed” button on the top left corner of the screen.

Now, a shortcode will appear on the screen. Copy that shortcode and paste it to the page or post you want the form to publish.

Click the “Publish” button and let’s check the form.

As you can see, the form displaying only a few fields on the first page. Once your users fill out the fields on the first page, they will be able to go to the next page by pressing the “Proceed” button.

And you are done with creating the multi-page form for your WordPress site.
